Telecare’s AB109 program is a Full Service Partnership (FSP). This county-contracted program is the result of a collaborative effort between Probation and Behavioral Health Services.
The Personal Service Coordinator II provides direct client community support and case management services to assigned clients. The individual will provide education and support to clients and their familie while collaborating with services and agencies such as the Conservator’s Office, Public Defender’s Office, Child Protective Services, Probation Department, California Department of Corrections and Rehabilitation and County Mental Health Services.
-High School Graduation or G.E.D. equivalent and four years of direct services in Mental Health; or Associate Arts degree and two years of direct services in Mental Health; or BS degree in Social Sciences and one year experience in Mental Health Services are required
-Experience in mental health setting, outpatient clinic or community mental health service preferred
-Experience in providing intensive case management, intensive community supporting/using psychiatric rehabilitation concepts is highly desirable
-Sensitivity to multi-cultural populations and issues is required
-Necessary skills include the ability to work effectively as a member of a multidisciplinary team, a thorough understanding of client rights, an understanding the conservatorship process, basic computer skills and knowledge of basic medical and psychiatric terminology
-Must have a valid California driver’s license, a DMV clearance for insurability, a vehicle (approved for safety) and proof of insurance
-Applicant must be fingerprinted and receive clearance from the Department of Justice and the Office of Inspector General
-Must have completed first aid and CPR training or obtain certification within four weeks of hire
