Telecare operates In-Home Outreach Teams (IHOT) in the North, Central and East County regions of San Diego County Health and Human Services Agency. An IHOT is a team that provides in-home outreach and engagement services to individuals with serious mental illness who are reluctant to seek treatment and to their family members or caretakers. IHOT teams consist of trained clinicians, family and peer support staff. IHOT teams provide in-home assessment, crisis intervention, case management, education and linkage to resources in the community. The program uses creative approaches establishing trust and human connection with IHOT consumers. The ultimate goal is to effectively connect IHOT consumers to ongoing outpatient care so that they may avoid unnecessary hospitalization and can achieve full and productive lives. POSITION SUMMARY Works to engage family members and the prospective participant with the goal of fostering a willingness to accept available services. Provides direct participant community support and case management services to assigned participants; works closely with participants and their families to ensure that their needs are expressed and that they have a “voice” in treatment decisions and the overall treatment process; in conjunction with the Team, completes Program assessments, runs groups, promotes engagement and provision of peer and family support services; an advocate for the participant and family for services as a point of interaction between participant’s family and the Program. QUALIFICATIONS The family coach must have lived experience helping their loved one maneuver through the mental health system. They must have parent, child or sibling with mental illness. Current or prior experience as a care-providing family participant of an individual with a mental illness required. At least one year personal experience within a mental health services environment or system required. Sensitivity to multi-cultural populations and issues. Must be able to incorporate creativity, invention and imagination in the daily execution of this role. Ability to function independently, analyze, interpret, and draw conclusions from varying facts and circumstances. Able to make decisions while maintaining awareness that this decision-making will have an immediate impact on the well being of the family of the participant and the relationship between the program and the family. Bi-lingual/Bi-cultural preferred. Able to work flexible schedule (outside normal business hours). Must have valid California driver’s license, a DMV clearance for insurability, an available vehicle (approved for safety), and proof of insurance.
