Job Description:

TELECARE CORPORATION Stults House Stults House is a 5-bed Residential Treatment Home located in Portland, Oregon. It serves people diagnosed with a mental illness who need support where they live due to long periods in institutional settings. Specifically, the program assists residents with ADLs, household responsibilities, establishing and maintaining personal relationships, conducting personal business in the community such as shopping, banking, accessing medical and social services, establishing leisure pursuits, vocational assistance and educational support (for those interested), and other life skill areas related to re-integration into the community. Stults House is named in remembrance of Dan Stults, a compassionate advocate for people with mental illness. In his honor, the program operates using the principles of Telecare’s Recovery-Centered Clinical System where partnership and power-with is emphasized. Since residents of Stults House may have a history of behavioral incidents due to living in institutional settings, the program takes a new and exciting clinical approach. Staff and residents work together to maintain a culture that honors respect, individual uniqueness, motivation, and personal power to build strong, trusting relationships. In this culture, individuals learn to make choices that steer their lives toward a future of their choosing. They explore who they are and who they want to be and then take steps to bridge the gap. Dreams for the future are established and the journey toward those dreams celebrated making Stults House a place of hope, energy, and companionship. PERSONAL SERVICE COORDINATOR I – STATEMENT OF PURPOSE We exist to help people with mental impairments realize their full potential. MISSION STATEMENT Telecare will deliver and manage excellent services and systems of care for persons with serious mental illness. POSITION OBJECTIVE Personal Service Coordinator I performs a variety of duties such as assists clients with the process of transitioning from hospital or crisis residential services to a community living setting involving client management and care. QUALIFICATIONS One (1) year of experience working with the severely and persistently mentally ill population, (1) year experience with working in a community setting, OR is a consumer of mental of mental health services and has demonstrated ability to perform at a consistently high level as a Personal Service Coordinator is required (PLEASE SPECIFY ON RESUME). High school graduation or a G.E.D. equivalent is required. Necessary skills include knowledge of psychology or sociology, knowledge of mental illness behaviors and treatment principles of interviewing to acquire information and the ability to interact with other members of the team, clients, family members and community services. The ability to read, write & speak English is crucial along with basic math skills. Must have valid Oregon driver’s license, a DMV clearance for insurability, a vehicle (approved for safety), and proof of insurance; must have knowledge and ability to use public transportation. Applicant must be finger printed, sign under penalty of perjury that there has been no conviction of any crime, other than minor traffic offenses, and receive clearance from the Department of Justice.
