Part of the Telecare family, this innovative Coordinated Care program helps people with a mental illness lead healthier and more fulfilling lives, by building their skills and abilities to self-manage health care and engage natural supports. The interdisciplinary team provides excellent, relationship-based care for up to 45 individuals who, in addition to mental illness, may also be grappling with a physical health condition and/or substance use. The Telecare program staff will work in collaboration with the Inland Empire Health Plan and community providers to systematically offer a variety of evidence-based strategies to empower and measurably increase the health and wellness of the program members. STATEMENT OF PURPOSE We exist to help people with mental impairments realize their full potential. MISSION STATEMENT Telecare will deliver and manage excellent services and systems of care for persons with serious mental illness. POSITION OBJECTIVE The Team Leader coordinates the day-to-day functioning of the Team, ensures and/or provides direct services and works with the Clinical Director in developing and implementing policies, procedures, and services. Required Skills QUALIFICATIONS Three (3) years of direct service experience with psychiatric consumers in an inpatient or outpatient setting is required. Two (2) years experience in supervision or administration of case management services. A Master’s Degree in Social Work from an accredited graduate school of Social Work is required. A license by the State of California as a Licensed Clinical Social Worker or Licensed Professional Clinical Counselor is required. Necessary skills include the ability to: thoroughly understand and communicate with the team about member rights and the conversatorship process; understand and communicate using basic medical and psychiatric terminology; partner well with people in the community with a positive, flexible, strengths-based, and solution-focused attitude; collaborate with substance use and healthcare providers; communicate effectively verbally and in writing; and collaborate effectively as a member of a culturally competent multidisciplinary team. Specialized knowledge/experience in dual diagnosis and/or integrated health services is preferred. Bilingual abilities preferred. Must have valid California driver’s license and access to a car and have the knowledge and ability to use public transportation. Must be CPR Certified. Applicant must be finger printed and receive clearance from the Department of Justice. Acceptable results for pre-employment physical is necessary.
