JOB TITLE: Team Leader – (Licensed/Licensed Waivered) Starting Salary range – Un-Licensed ($41,800 – $47,000 + Benefits) Licensed ($48,100 – $54,100 + Benefits) Program Context: On April 21, 2015, San Diego county Board of Supervisors approved implementation of “Laura’s Law” (also known as Assisted Outpatient Treatment – AOT), a measure that allows courts to order certain mentally ill and violent people into compulsory outpatient psychiatric care. Telecare’s extensive experience as a provider of Assertive Community Treatment (ACT) services, our excellent long-standing working relationships with County stakeholders, and our specific recent experience implementing programs related to Laura’s Law all contribute to our enthusiastic interest in this project. Program Opening: Program is scheduled to open to new members / patients at the end of March 2016. Ideally staff will start work on or around 3/15/16. POSITION SUMMARY Provides clinical and administrative supervision, acts as team leader, provides coaching and education for assigned team members; completes assessments and determines level of services for new members. Program Scope (opening at the end of March 2016): The purpose of this project is to implement ACT Team services to a caseload of 114 people (200 per year), who have been diagnosed with serious mental illness and have been referred by the In-Home Outreach Team (IHOT). Each individual referred is a potential AOT candidate who voluntarily accepts this service, a person with AOT court settlement status, or a person with court-ordered AOT status resulting from a contested hearing. About Telecare: MINIMUM QUALIFICATIONS Licensed or licensed eligible mental health professional with two (2) years of direct adult clinical service is required. To be licensed eligible, candidates must have an intern number for the state of California (MFTI, ASW, or LPCC intern). Outpatient clinic or community mental health service is preferred. One (1) year supervisory experience is also preferred. Experience in providing intensive case management, intensive community supports using psychiatric rehabilitation concepts is highly desirable. A current LCSW, MFT, LPC, or Ph.D. valid license or ASW, MFTI, LPCC Intern license eligibility is required. Sensitivity to multi-cultural populations and issues is required. A valid California driver’s license, a MVR clearance for insurability, a vehicle (approved for safety), and proof of insurance is required. Applicant must be finger printed and receive clearance from the Department of Justice. Must have completed first aid and CPR training or obtain certification within four weeks of hire. Proficient in Microsoft computer applications and experience with an electronic health record preferred. PERFORMANCE STANDARDS QUALITY AND QUANTITY OF WORK Directs the day-to-day clinical operations of assigned team, assures all members receive services as indicated by Treatment Plan. Provides consultation/interpretation to members and their families regarding issues arising from the members’ treatment. Observes, reports, and documents member social and psychiatric behavior. Directs the day-to-day clinical operations of assigned team, assures all members receive services as indicated by Treatment Plan. Develops and maintains an awareness of community resources and makes appropriate community referrals. Provides education/training to other team members as necessary. Interacts professionally with community providers and peers. Encourages member utilization of community based resources.
